(NYT21) NEW YORK -- Oct. 16, 2005 -- MEDICAID-CLINIC-2 -- At a Morris Heights Health Center pediatric clinic, Isiah Brown, 5, waiting with his grandmother for a physical so he can attend kindergarten in September 2004. The center, on Burnside Avenue in the Bronx, serves 45,000 patients a year, many of them on Medicaid. Sixteen months spent inside Morris Heights, listening to patients, clerks, nurses, doctors and administrators, provides a stark education in Medicaid as it is lived at coarse, unpredictable ground level. Those months illuminate Medicaid's sprawling good works and good intentions, and how they are undermined by inscrutable rules, daunting paperwork, human frailties and, plainly, the puzzling ways New York spends the program's billions of dollars.
