This family photo shows Carol Svec in her Raleigh, N.C. home, June 16, 2005. Svec is one of about 4,000 patients whose surgical instruments were incorrectly washed in hydraulic fluid instead of detergent at the Duke Health Raleigh Hospital, in Raleigh, N.C. Svec had surgery in Nov. 15, 2004, to repair a torn rotator cuff in her left shoulder. The fluid, which looks similar to the soap used to clean the instruments, was drained from an elevator system and put into 15-gallon soap containers and recapped without new labels. State investigators cited the hospitals and the elevator company for mistakes that created the confusion, including poor communication and improper labeling of chemicals. (AP Photo/Family Photo)
